Relationships of Mental Health Conditions to Type 2 Diabetes Outcomes

Abstract

Patients with type 2 diabetes mellitus require self-management skills to control their blood sugar. Patients who also have mental health conditions may experience more difficulty in controlling their blood sugar than those without mental health conditions. Others have studied whether comorbid diagnoses of certain mental health conditions, such as depression & anxiety, serious mental illness, and substance use disorders, relate to blood sugar control. However, results were mixed on whether the particular mental health condition was associated with worse blood sugar control – some results pointed to worse control, and some to better. The previous studies were based on small datasets. Here, we use a larger, more comprehensive dataset (N=3665 from the Arkansas Clinical Data Repository) to better understand how mental health conditions in general, and partitioned into types, relate to blood sugar control. We comment on implications for clinical pharmacists.

CDC Director Discusses Important Public Health Issues for Arkansas

Stigma has no role in public health, said Robert Redfield, M.D., director of the Centers for Disease Control and Prevention during a visit June 27 to UAMS.

Speaking on “Partnering for a Healthier Arkansas,” he addressed ending epidemics, eliminating diseases, and securing global health and ensuring domestic preparedness.

Redfield presented to a room full of UAMS faculty, staff, and students in the Walton Auditorium. The two epidemics Redfield talked about were reducing opioid misuse and antimicrobial resistance.

“Addiction is a medical condition,” said Redfield. “We need to approach it the same way we do other diseases.”

He emphasized creating a climate that allows people to get the medical care they need. He discussed the value in having real time data solutions such as syndromic surveillance — gathering health-related data before a diagnosis to signal the likelihood of a public health issue —to help combat problems like the opioid epidemic. Arkansas is currently gathering such information from hospitals and clinics throughout the state

Redfield also discussed how to eliminate HIV, saying “Fifteen percent of people with HIV in Arkansas have not been diagnosed yet.”

Arkansas has 291 new cases of HIV diagnosis each year, which is the 20th highest rate in the country.

“We have the tools we need to end this epidemic in the U.S.,” said Redfield. “We can diagnose and lay out a treatment plan for those with HIV.”

Redfield discussed prevention interventions, like PrEP or Pre Exposure Prophylaxis, a medication that can be given to those at high risk for being exposed to HIV through sexual contact or injecting drugs.

“If we don’t do anything about this epidemic today, we can anticipate another 400,000 people being diagnosed each year, costing $200,000 million,” said Redfield.

He added that doing this now shows the value in putting science into action.

Redfield also discussed President Donald Trump’s HIV initiative challenging the country to reduce new infection by 75% in the next 5 years.

At a news conference earlier in the day, Redfield announced that Arkansas will be a recipient of funding tied to this new initiative due to its high rate of HIV infection in the country.

He also talked about ways to prevent vaccine preventable diseases. He challenged health care professionals to spend more time helping parents understand the risk of not vaccinating their children.

He said the most important tool is to not underestimate the threat to society posed by people not getting vaccinated.

As CDC director, Redfield oversees the nation’s health protection agency. The CDC serves as the national focus for developing and applying disease prevention and control, environmental health, and health promotion and health education activities designed to improve the health of the people of the U.S.

He succeeds founding Dean Jim Raczynski, Ph.D., who stepped down Dec. 31. Jay Gandy, Ph.D., professor and chair of the Environmental and Occupational Health Department, had served as interim dean.

“On the anniversary of the College of Public Health’s July 1, 2001 establishment, it is fitting to welcome Dr. Williams to lead the college that in not quite 20 years has created such momentum around public health approaches to address our state’s health needs,” said Stephanie Gardner, Pharm.D., Ed.D., provost and chief strategy officer. “Dr. Williams brings wide experience in public health education and research — combined with enthusiasm toward our health improvement mission — that will benefit Arkansas.”

Williams’ public health career spans over 30 years, having published over 200 peer- reviewed articles and chapters. His research focuses on behavioral epidemiology, disease prevention, and health promotion in underserved and minority populations.

“I am honored to be selected as dean of the College of Public Health and look forward to leading the college as we seek to improve the health and well-being of all Arkansans through education, research and service,” Williams said.

Williams comes to UAMS from the Robert Stempel College of Public Health and Social Work at Florida International University, where he held multiple roles. From 2011 to 2013, he served as professor and chair of the Department of Health Policy and Management. He was appointed associate dean of Academic Affairs in 2012 and interim dean from 2013-2016.

Prior to that Williams served in academic roles at the University of Texas School of Public Health, the University of St. Thomas, and the University of Rhode Island. He worked in the private sector as vice president for behavioral research at NOVA Research Company and vice president of research and development at Affiliated Systems Corporation.

Williams was one of the first to study the epidemiology and prevention of HIV in drug users and was an original contributor to the development of community-based prevention interventions with this population. His scholarly work includes studies of HIV transmission in injection drug users, disease transmission in social networks of drug users and men who have sex with men, and HIV prevention.

His research group was the first to establish injection drug use as a mechanism of HIV transmission in sub-Saharan Africa. Also, his work was the first to establish the effectiveness of brief internet interventions to prevent HIV transmission among gay and bisexual men in rural areas.

Williams received his undergraduate degree from the University of Iowa in 1976 and his Master of Arts degree from the University of Nebraska in 1979. He received his Doctor of Philosophy degree in 1983 from the University of Iowa.
